Financial Declaration Form

For dancers under 18, this form must be completed by a parent or guardian. A dancer aged 18 or over may complete the form themselves.

Production Dancers 26/27 Fees and payment schedule

Fee: £90 per month for four monthly payments

(total £360)

Payments are due on the 1st of each month,

beginning 1 September 2026 and ending 1 December 2026.

Notice: Three months’ written notice is required from the date notice is given. Notice must be sent by email to mkdancetheatre@gmail.com.
