For dancers under 18, this form must be completed by a parent or guardian. A dancer aged 18 or over may complete the form themselves.

Photography & video consent

Consent is required for participation.

I give MK Dance Theatre permission to photograph and film the dancer during MK Dance Theatre activity, and to use these photographs and videos on the MK Dance Theatre website and social media channels for promotional purposes.
