Please complete all forms that apply to your dancer. For dancers under 18, forms must be completed by a parent/guardian; dancers aged 18 or over may complete their own forms.

This is a private central space for parents and guardians of our current MK Dance Theatre dancers. Here, we'll share important updates, key dates, rehearsal and performance information, reminders, and other useful announcements throughout the year.

The hub will also help us gather essential information, including sign-up forms, emergency contact details, financial declarations and consent forms.

Access is limited to parents & guardians of current MKDT Dancers, helping us to keep communication clear, relevant and secure.
