Acceptance Form

For dancers under 18, this form must be completed by a parent or guardian. A dancer aged 18 or over may complete the form themselves.

Acceptance, Attendance & Code of Conduct

Attendance: The dancer must maintain at least 80% attendance across scheduled sessions. All announced performance dates are required.

Respect: Treat fellow dancers, staff, volunteers, visitors and audiences with kindness and respect. Bullying, harassment, discrimination, abusive language and intimidating behaviour are not acceptable.

Professional conduct: Arrive punctually, prepared and in suitable attire. Follow artistic direction and all safety instructions.

Care: Treat venues, equipment, costumes and other people’s belongings carefully. Respect the shared rehearsal and performance environment.

Communication: Notify MK Dance Theatre promptly of absence, injury, welfare concerns or anything that may affect participation. Communicate respectfully in person, WhatsApp messenger or Email.

A serious or repeated failure to meet this code of conduct will be reviewed by MK Dance Theatre.
